113th Academic Year Admission Guide for Master's Students

1.Thesis Advisor Consent Form (Download)
Instructions: Graduate students must select a faculty member of assistant professor rank or higher from the department to serve as their thesis advisor within one week of enrollment, subject to the approval of the department chair. Please submit the form to the department office by noon on September 12 (Thursday).

2.Application for Evening and Weekend Laboratory Access (Download)
Instructions: If you need to use the laboratory after 18:00 or on weekends for project or research purposes, you must fill out and submit this form in advance. Please submit the form to the department office by noon on September 12 (Thursday).

3.Course Credit Waiver Application Form (Download)
Instructions: Please refer to the 113th Academic Year (2024-2025) Curriculum Chart (Download) when filling out the form, and have it signed by the course instructor. Please submit the form to the department assistant by 5:00 PM on September 18 (Wednesday) for collective submission to the department chair and the dean for approval.
